By signing this form, I give Loving Paws Animal Center, including their TNR volunteers, permission to be on my premises to assess the cat situation and set traps according to scheduled appointments.

I will not hold Loving Paws Animal Center responsible for any damages caused by owned or community cats on my property.

I understand that a cat's pregnancy will be terminated and the cat spayed.

I understand that ALL cats will return with an ear tip to indicate that he/she has been spayed/neutered.

I understand that all cats will be returned to the location they were trapped at, and Loving Paws Animal Center will NOT remove/relocate any cats.

Cats deserve to remain in his/her known location, allowing them the best chance at survival. Kittens will be removed on a case by case basis, only when foster placement is available.

I understand that Loving Paws Animal Center is not responsible for any unforeseen complications during surgery, including rare occurrence of death.
